We are building a consortium in the area of lung, heart, artieral ultrasound data sets on covid for cloud-based monitoring. We are seeking ultrasound data for heart, lung, and arterial which will help solidify our ai system design. We already have italy, canada, usa, and china as partners... I am trying to get india, africa, and europe and middle east. The image data needs to be collected for patients, such as carotid, heart, lung, liver, brain, etc... These patients have comorbidity, this means they have multiple diseases besides covid-19. These patients are covid-19 patients but had a pre-existing disease like diabetes, coronary artery disease, liver disease, heart disease, neurological disease and are probable high age groups. We are looking datasets for 3,500 patients. The imaging data must be with the diagnostic centers or hospitals for these dead patients or patients who are in icu and must have undergone the imaging. We need imaging data for tissue characterization using ai. We need these datasets for dead, living patients with mild covid-19 or patients who are alive with severe covid-19 condition. I would like to speak to an expert with the way cms expects to track hospital wide readmissions from a quality standpoint. Biggest challenge in accurately capturing readmissions many institutions' it systems require transfers within the hospital to and from a non-acute care center (hospice psych, swing beds, rehab, icu unit etc) to be coded as a separate discharge or admission. These are not true readmission and must be excluded from the readmission count. We would like to know what is the recommended approach to exclude these cases? Would it be using combination of discharge disposition codes and revenue codes. If yes, is there a set of codes we can use? Second question is about the planned readmission that cms recommends that can be excluded. Cms uses ahrq ccs (inturn icd codes) categories to define these planned readmission. Need advice on applying these exclusions to the population of discharges. Should it be checked against an index discharge or readmitted discharge or any discharge regardless of whether index or readmission? Which is the discharge that needs to be excluded here? The discharge that satisfies the condition or the index discharge corresponding to the readmitted discharge that satisfies the condition?
