Ischemic Stroke
72 year old woman has ischemic stroke and is hospitalized. The following day she is nearly back to baseline. A resident starts her on heparin at a dose higher than the institution's protocol. (she had been on coumadin for a-fib.) she has an aptt of 176 and heparin is held for an hour but then re-started at same dose, again in violation of protocol. Woman has a massive brain bleed and severe brain damage that is too severe to operate. She dies and family is told by attending neurologist that resident did not understand protocol.
